Participate as one of our Flu Shot Clinic Location

If you are a licensed health care provider and own or operate a health care facility, and if you do offer flu vaccinations, you could be designated as a Flu Clinic Location of the Flu Shot Center. There is no cost to you to participate. Our policy is to limit  locations to one in each particular distance from another. Therefore, those who register first are considered first for this option. What do you need to do to be a Flu Shot Clinic location?

All you need to do is to click here to register with us. We would shortly send you an email confirmation and our staff will contact your office or business to arrange for that. We will fax you a consent form for your information and signature in order to list your office. Also, we will need to know the hours that your office is planning to allocate to flu vaccination offered to public and what you will charge a prospective recipient for this service.
